The Biosecurity Horse Health form is required for all transport of livestock.
Forms must be completed, signed and handed in on the day of the event to the Show Secretary/ Treasurer.

WAIVERS & WRISTBANDS
DA National Risk warning & Waiver of Liability form must be completed and signed by Every person/s entering the Show ring NO EXCEPTIONS.
ALL Financial DA Members are covered by our insurance paid in your membership fees; however, you still need to compete and sign a waiver for each show.
Please COMPLETE AND SIGN the sections highlighted in yellow.
All Non-DA Members MUST sign a Waiver & pay $15.00 non-member fee and wear a QB DA provided Wrist band to enter the Show Ring.
All signed Waiver can be handed in on the day or emailed with Show Entry/s to Show Secretary.
